2025 CapABLE Conference: A Piece of the Pi – Sweetening the Pediatric to Adult Transition Journey

Please join us for our annual NCH CapABLE Employee Resource Group Transition Conference and Resource Fair. In 2025, Educational content will be offered for healthcare providers who support individuals with disabilities as they transition from adolescence to adulthood. Presentation topics will cover medical transition, supported decision-making, caregiver supports for fostering independence for their adult children, and providing a structured, person-centered planning tool for individuals with disabilities moving into adulthood. Attendees will also have the opportunity to visit and gain knowledge from community vendors throughout the resource fair. We look forward seeing you!

In support of improving patient care, Nationwide Children's Hospital is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the American Nurses Credentialing Center (ANCC),  and the Accreditation Council for Pharmacy Education (ACPE), to provide continuing medical education for the healthcare team.

This conference is offering continuing education credits for Service and Support Administrators (SSA).

Cancellation Policy

Cancellations received in writing no less than 20 days before the course will be refunded, less a 20% administrative fee. No refunds will be made on cancellations received after that date. Please send cancellation requests to Whitney.Phillips@NatiownideChildrens.org.

Nationwide Children's Hospital reserves the right to cancel or postpone this program if necessary; in the event of cancellation, course fees will be fully refunded. We are not responsible for other costs incurred such as non-refundable airline tickets or hotel penalties.

Scholarships Available

We have scholarships available to assist with registration costs for family members of individuals with disabilities as they transition from adolescence to adulthood. If you require financial assistance, please email Whitney.Phillips@nationwidechildrens.org.

Note: Scholarships are only available to registrants who are not seeking continuing education (CE) credit.
