  • 5 K23 GM104354-05: Association of childhood obesity with early post-tonsillectomy pain NIH-DHHS-US-13-PAF05168 Nafiu, Olubukola, PI 09/2014-08/2019. $954,358 ($193,195)

I successfully competed for a five-year National Institute of Health (NIH) career development (K23) grant in 2014. This funding from the NIH was a career development award. The research objective was to examine the role of preoperative dexamethasone on the post-tonsillectomy pain experience of children. This was an important achievement for a number of reasons. First, to secure funding during a time of extremely stringent NIH budget restrictions is testament to the quality of my research and the importance placed by the NIH on such. Secondly, as the first ever funded K23 grant in the Anesthesiology department at Michigan, this reflects the quality of my research and is another way that my work continues to improve the image of Anesthesiology at the local and National level.

Despite the well-described secular trend in the prevalence of childhood obesity, prior to this article, there had been no previous description of overweight/obesity prevalence in a pediatric surgical population. This work helped to spotlight the prevalence of high body mass index among children undergoing elective non-cardiac operations and it prompted discussions about the need for BMI measurement in the perioperative setting.

2. Nafiu OO, Reynolds PI, Bamgbade OA, Tremper KK, Welch K, Kasa-Vubu JZ. Childhood body mass index and perioperative complications. Paediatr Anaesth. 2007;17(5):426-30.

 

This continuation work solidified my role as a foremost childhood obesity researcher. We showed for the first time in the anesthesia literature that high body mass index was a risk factor for a variety of perioperative adverse events in children. This article documented increased incidence of respiratory complications among overweight/obese children compared to their lean peers. There was considerable interest in the lay press (New York Times, LA times and Reuters) about the findings of this article. These findings have since been confirmed by other investigators and the paper has been cited several times.

3. Nafiu OO, Burke C, Lee J, Voepel-Lewis T, Malviya S, Tremper KK. Neck circumference as a screening measure for identifying children with high body mass index. Pediatrics. 2010;126 (2): e306-10.

Recognizing the low precision of body mass index as an index of adiposity, we designed this prospective observational study to determine the ability of neck circumference measurement to correctly identify children who were overweight/obese. This article showed that wide neck circumference measurement had high sensitivity and specificity in identifying children who were overweight/obese. It was the first time this was being done in a US pediatric population and there was an accompanying editorial as well as several pieces in the lay press (LA times, Reuters, CNN and the Detroit TV).

4. Nafiu OO, Burke CC, Gupta R, Christensen R, Reynolds PI, Malviya S. Association of neck circumference with perioperative adverse respiratory events in children. Pediatrics. 2011; 127 (5):e1198-205.

This follow-up article from my group explored the association between indices of central adiposity and perioperative respiratory adverse events. The impetus for this study came from the observation that although body mass index is the most widely used measure of obesity status, it was generally poor in producing estimates of obesity-related chronic risks. Given the increasing evidence that central obesity was more pathogenic than peripheral adiposity, we sought to determine whether neck circumference measurement as an index of central obesity could be used to predict some perioperative complications in children. We demonstrated for the first time that increasing neck circumference measurement in children was significantly associated with perioperative respiratory complications thus providing a simple clinical tool for pediatric perioperative risk stratification.

This article reflects my interest in identifying simple clinical symptom/sign to stratify children at risk for perioperative adverse events. In this prospective, cross-sectional study, we identified habitual snoring as an important symptom to explore in the preoperative evaluation of children undergoing elective, non-cardiac operations. We showed for the first time in the anesthesia literature that habitual snoring was an independent predictor of perioperative complications in children.

6. de Graaff JC, Pasma W, van Buuren S, Duijghuisen JJ, Nafiu OO, Kheterpal S, van Klei WA. Reference Values for Noninvasive Blood Pressure in Children during Anesthesia: A Multicentered Retrospective Observational Cohort Study. Anesthesiology. 2016;125(5):904-913.

 

This seminal article reflects my interest in perioperative hypotension. Following an earlier series of publications on incidence and risk factors for perioperative hypotension in children, we convened an international group to determine normative reference values for intraoperative blood pressure in children. To date, this is the only paper on the refence values for intraoperative blod pressure in children and the paper is highly cited.

Specific contributions: I was a major contributor to the study design, manuscript drafting, revision and eventual publication.

 

7. Nafiu OO, Chimbira WT, Stewart M, Gibbons K, Porter LK, Reynolds PI. Racial differences in the pain management of children recovering from anesthesia. Paediatr Anaesth. 2017;27(7):760-767.

This article ignited my interest in healthcare disparity research. Using a prospective, observational design, we sought to determine whether race was a factor in the recovery room treatment of pediatric pain. We showed, for the first time in the anesthesia literature, that although black children were more likely to have higher pain scores than their white peers following surgery, there was no significant difference by race, in the likelihood of postoperative pain treatment among our study cohort.  This negative study was important because it underscored the cultural difference in pain treatment between the recovery room and the emergency department where previous investigators have demonstrated racial disparity in pediatric pain treatment.

8. Nafiu OO, Mpody C, Kim SS, Uffman JC, Tobias JD. Race, Postoperative Complications, and Death in Apparently Healthy Children. Pediatrics. 2020;146(2):e20194113.

 

This is arguably one of my most consequential publication and it solidified my role as a foremost healthcare disparity investigator. We showed for the first time, that even among relatively healthy pediatric surgical cohort, black children were about three times more likely to suffer postoperative mortality and they also had higher rates of post-surgical complications. There was considerable interest in the lay press (New York Times, LA times, Washington Post, CNN, Reuters and the local TV stations) about the findings of this article. I have been invited to give several lectures at local, national and international platforms since the publication of the article.

This was a follow-up study that further highlighted racial disparities in the distribution of major postoperative pulmonary complications among children undergoing otolaryngology procedures. We showed that black children undergoing ENT procedures were at higher risk of major perioperative pulmonary complications and recommended targeted preventive measures for these children.
